Health Forms
Full physicals are required by Illinois Law for all children entering school for the first time, entering preschool, kindergarten, or sixth grade, or transferring from another state or country.
All new students must have an updated immunization record. They must be filled out, signed by the doctor and returned to the school office. Dental exams are required for Kindergarten, Second, and Sixth Grades. According to Illinois law, any child whose parent has not presented proof of the required health examinations and immunizations shall be excluded from school. Parents shall meet these requirements by the first day of school. After that date, if such proof has not been presented, the child will be refused admittance.
